Wellness Clinic

The Horizon House Wellness Clinic is open to serve residents seven days a week.  Our house staff physician works with the other team members: Wellness, Physical, Occupational and Speech Therapies; Nursing; and Dietary Case Management, in enabling each resident to regain and to maintain the highest level of functioning.

An on-site Dental Clinic offers a full range of care.

Podiatrist visits are scheduled monthly.

The Clinic also provides assistance with clarification of insurance benefits and Medicare billing for staff physicians during on-site visits.

Prospective Horizon House residents and their families receive thoughtful, compassionate case management as they work through the changes and transitions in their lives. We can direct the family to home care services and community resources such as support groups and service organizations.

In an advocacy role, we clarify resident rights and ethical issues, and we extend particular support for residents with limited family or social support systems.

The family plays an important role in the resident's plan of care through family conferences, consultation and discussion of the resident's individual needs.
