Supported
Living is a unique program of
individualized care that is designed to fluctuate and change as your
needs change.
Welcome to the Horizon House
Supported Living services page. From light assistance with daily living to nursing care, our
services are designed to meet the changing needs of our residents. The
following is a description of services that comprise the basic service
package provided to all residents in Supported Living.
Basic Service Package
Nursing
Services
Nursing
staff are on duty 24-hours per day.
All residents receive a comprehensive assessment performed by a
licensed nurse, development and regular review of a negotiated service
agreement, quarterly assessment and
review of medication, quarterly review
of physician orders and assistance with arranging medical
appointments. General
oversight of health and wellbeing including monthly monitoring of
vital signs, pulse, temperature, weight are also included in the basic
service package.
Activities
Daily
activities programs are offered in Supported Living.
These include group and individual activities such as spiritual
care programs, horticulture programs, cooking programs, games, word
games, travel programs, exercise and outings.
A monthly calendar is available describing these programs.
Resident interests are assessed and residents receive reminders
and encouragement to attend programs of interest.
Social
Services
Social
Service support is available to each resident to assist in
understanding and meeting their individual care needs.
Social Service staff members are available to help facilitate
admissions and discharges from supported living and to identify
additional support services when needed.
Dietary
Services
Three
nutritionally balanced meals per day are provided to residents in the
Supported Living dining rooms. Additional
snacks are readily available. Each
resident receives a nutritional assessment by the Horizon House
Dietetic Technician. Special
diets as ordered by a physician are provided.
Medical
Records
A
medical chart is maintained for each resident.
This chart contains the resident assessment, negotiated service
agreement, staff documentation, physician orders and documentation,
advanced directives and other information pertinent to the delivery of
resident care.
Housekeeping
Each
resident room is cleaned daily including trash emptying, spot
vacuuming and cleaning of the bathroom.
Weekly deep cleaning of resident rooms includes wall-to-wall
vacuuming, dusting, cleaning of the bed and room vents.
Laundry
Services
Regular
linen service including bed linens and towels is provided weekly or
more often as needed. Two
loads of personal laundry per week are provided to each resident as
part of the basic service package.

Additional
Services
Additional
services are available and are applied to the level of care based on
the amount of staff time required for each service.
Nursing
Additional
nursing services such as medication administration, support for
diabetic management and physician ordered medical treatments are
available.
Support
for Activities of Daily Living
Nursing
assistants are on duty 24 hours a day and available to assist with
activities of daily living such as dressing, bathing, toileting,
grooming and mobility assistance.
Each resident will be assessed by the nurse for the level of
support needed.
Dietary
Assistance
with dining and room tray service are available.
Laundry
Additional
laundry services are available including extra loads of laundry and
ironing.
Housekeeping
Additional
housekeeping services are available.
