Horizon
House affirms the desire of residents to remain independent and to “age
in place.” Residents in need of additional support often call upon the
help of family members or private caregivers.
Horizon House now offers Adult Day Services for residents with
physical or cognitive challenges, or who are experiencing social
isolation, fresh opportunities to enjoy companionship, become immersed in
meaningful activities, and engage in special events such as musical
programs. Adult Day Services
will also provide assistance for caregivers, especially spouses, who can
attend to other aspects of life secure in the knowledge that loved ones
are receiving excellent supplemental care.
Adult
Day Services are available through Supported Living in the Memory Support
Neighborhood, Monday through Friday, 9 a.m. to 3:30 p.m.
Residents may participate in Adult Day Services from one to five
days a week, in the most beneficial time period during hours of operation.
For example, one resident might participate two days a week for five hours
each time while his spouse does chores or shopping and enjoys lunch out
with a friend. Another
resident might participate five times a week for two hours a day, so that
her spouse is able to engage in an exercise routine.
Charges
cover initial and ongoing assessment, staff support including nursing,
social services, activities and medical records, and lunch or snacks as
needed during the time of attendance. The Half
Day fee is $35 for three hours or less, and $60 for a Full Day period of
