Our Supported Living program offers both long-term residency, as well as short-term respite care. Horizon House residents whose level of care has changed may select to move to one of our two Supported Living floors, but prior residency is not required. Apartments are rented on a month-to-month basis, and there is no Entrance Fee.
- A resident is preparing for a hospital procedure
- An illness, a fall, or frailty indicates that a resident is no longer comfortable or able to feel safe at home
- A spouse or family member has a planned vacation or emergency
- An individual requires ongoing assistance with self-care tasks, medication management, and monitoring of medical conditions such as heart disease, Parkinson’s, or other chronic illnesses
- An individual is isolated and would benefit from a caring atmosphere and added socialization
- An individual has limited energy to maintain living independently
- A spouse is no longer able to provide care
- Private caregiver expenses have become unmanageable
We offer care under a comprehensive monthly service fee, which is paid for privately and can be supplemented through long-term care insurance. When hospitalization or short-term stays in a skilled nursing facility are required, rest assured that our licensed nursing staff stays in close contact with our residents throughout their stay. Horizon House maintains close relationships with nearby skilled nursing facilities who serve our residents on a priority basis for these short-term stays. Residents are closely monitored and a follow-up care plan is put into place upon their return to our community.

Personalized and resident-focused care.
At Horizon House, residents are encouraged, supported, and involved in making decisions about their care. The care provided is individualized to each of our resident’s specific needs and outlined in a detailed care plan. Each resident, along with their family, is involved in designing, customizing, and evaluating their care plan. This is achieved through frequent communication among the resident, the family, and the care team.
Comfortable environment.
Each resident has their own private studio or one-bedroom apartment with their own furnishings and belongings to make them feel right at home. Friends and family are always welcome to visit. Should a resident’s healthcare needs change, additional care will be provided right in their apartment – eliminating the need to move to receive a higher level of care. Most importantly, residents enjoy a home-like environment, and are encouraged to live as independently as possible. Residents are free to come and go, travel, and choose from many opportunities to be active and involved.
Our neighborhood concept.
Our neighborhood concept ensures that residents are continuously taken care of by the same caregiver – someone they can grow to know and trust. Residents become part of a neighborhood and receive care from someone who knows their needs and preferences. This continuity between residents and caregiver provides a sense of comfort and stability – forming a personal relationship among the resident, family members, and our licensed nursing staff.

Monthly fees start at $4,100 and are based on apartment size and level of care needed. The amount of care and support a resident requires is assessed upon move-in, as well as annually (and in the event of a significant healthcare change). A second person fee of $1,200 is applicable to all apartments.
